I am also running Adobe Reader 10, version 10.0.1. So, that's not it. I'm on the 32 bit version though.

Adobe Reader 10 (10.0.1) and the 32 bit version also, but I have the same error:

I uninstalled Adobe Reader 10 and installed Version 9.4 now the files work for me.

Oops... meant to reply sooner, but I needed a quick reread of the book to check up exactly what I was thinking about. And then I didn't reread any of the book until Saturday.
1) I had always thought that the slender/broad distinction held even for weak N, L, R.
2) I had no concept of "grinning" O and U, and my ao wasn't a grinning U (although I'm still not 100% clear on where grinning O arises).
Those are the only ones I spotted, but maybe I was skimming too quickly, as I have a feeling there's more....
