I don't know about the other two. We would need context for the last one anyway, because it's ambiguous.

With "over my dead body", something like:
Young son: "There's a new Ferrari that I want!"
Mother: "Over my dead body! Just look at what you did to my car last month!"

"What's-his/her-name", as in one whose name one does not remember, something like:
"I've forgotten about what's-his-name. Let's see, what was his story?"

OK, forget my tr*nsl*t**n of the second one then. What is the context of the third one?

They are all a bit over my head, but maybe someone else will have an idea.

"So-and-so", as in "Mr. So-and-so".
